RESEARCHARTICLE ExceptionalHeterogeneity in Viral Evolutionary Dynamics Characterises Chronic Hepatitis C Virus Infection

چکیده
The treatment of HCV infection has seen significant progress, particularlysince the approval of new direct-acting antiviral drugs. However these clinical achievements have beenmade despite an incomplete understanding of HCV replication and within-host evolution, especially comparedwith HIV-1. Here, we undertake a comprehensive analysis of HCV within-host evolution during chronic infection by investigating over 4000 viral sequences sampled longitudinally from 15 HCV-infected patients. We compare our HCV results to those from a well-studied HIV-1 cohort, revealing key differences in the evolutionary behaviour of these two chronic-infecting pathogens. Notably, we find an exceptional level of heterogeneity in the molecular evolution of HCV, both within and among infected individuals. Furthermore, these patterns are associated with the long-termmaintenance of viral lineages within patients, which fluctuate in relative frequency in peripheral blood. Together, our findings demonstrate that HCV replication behavior is complex and likely comprisesmultiple viral subpopulationswith distinct evolutionary dynamics. The presence of a structured viral population can explain apparent paradoxes in chronic HCV infection, such as rapid fluctuations in viral diversity and the reappearance of viral strains years after their initial detection.

متن کاملDetection of Pre-treatment mutations leading to resistance to direct hepatitis C virus blocking drugs in patients with chronic hepatitis C
Background and objective: Human is the only host of hepatitis C virus. This virus has a positive single stranded RNA and lipoprotein envelop that has 7 confirmed genotypes. According to studies, genotypes 1a, 3a and 1b are the most common genotypes in Iran. No effective vaccine against HCV infection has been developed instead, advances in antiviral treatment using drugs that directly affect spe...
متن کاملSevere muscle weakness during treatment with pegylated interferon alfa for chronic hepatitis C virus infection A rare complication
Background: Interferons (IFNs) are common therapeutics for several diseases such as viral hepatitis, multiple sclerosis and malignancy. A variety of autoimmune related side effects have been observed during IFN therapy. Rare cases of myopathy, polymyositis or dermatomyositis have been reported during therapy with high doses of IFN^5. Case Presentation: In this report, we describe a case with se...
متن کاملMolecular Detection of HGV RNA in Chronic Hepatitis Patients from Afzalipoor General Hospital in Kerman, Iran
Background and Aims: Viral hepatitis is a global health problem with a high mortality rate. Recently, a new Flavi-like virus, provisionally named hepatitis G virus (HGV), has been described. HGV does not induce an immune response that is consistently detectable by using recombinant proteins from prokaryotic expression, therefore studies have been conducted by using polymerase chain reaction (...
متن کاملExpansion of CD4+CD25+FoxP3+ Regulatory T Cells in Chronic Hepatitis C Virus Infection
Background: Regulatory T cells (Tregs) have been involved in impaired immunity and may have a pivotal role in persistence of viral infections. Objective: To develop a simple and reliable in-house three color flow cytometery of peripheral blood to understand the role of HCV infection in the increase of Tregs. Methods: The level of naturally occurring CD4+CD25+FoxP3+ regulatory T cells (nTregs) i...
